Client Rights & Responsibilities

Catalysts Helping Increase Potential (“CHiP”) is committed to protecting the privacy, dignity, and personal information of every individual we serve. This Confidentiality Policy explains how CHiP handles client information shared during services, appointments, referrals, and general communication.

This policy applies to all clients, staff, volunteers, and partnering agencies.


 1. Our Commitment to Your Privacy

CHiP values your trust. Any personal information you share with us — whether in person, by phone, through email, or during services — will be treated with the highest level of respect and confidentiality.

We only collect information that helps us understand your needs and provide services safely and effectively.


 2. Information We Keep Confidential

CHiP will keep the following information private and confidential:

  • Your name
  • Contact information
  • Personal history
  • Service needs
  • Housing status
  • Any sensitive information shared verbally or in writing
  • Details of counseling, mentoring, or support interactions
  • Case notes (if used)
  • Referral information
     

Your information will not be shared publicly or with outside organizations without your permission, except where required by law.


3. Who Has Access to Your Information

Only the following may access your information:

  • CHiP staff
  • Authorized CHiP volunteers
  • Partner agencies only with your consent
  • Legal authorities only when required

Access is granted solely for the purpose of providing support, referrals, or safety.


4. When Confidentiality Must Be Broken

CHiP may be legally required to release information only if:

  • You are in immediate danger
  • Someone else is in danger
  • A child, elderly person, or vulnerable adult is being harmed
  • We receive a lawful request from authorities
  • Safety must be ensured for you or other

These situations are rare, but protecting life and safety always comes first.


5. How Your Information Is Used

Information you share may be used to:

  • Provide services
  • Assist with referrals
  • Coordinate support with partnering agencies
  • Ensure safety
  • Document progress (if applicable)

Information is collected only to help support your needs.

 

6. How We Protect Your Information

CHiP takes reasonable steps to safeguard client information, including:

  • Limiting access to authorized staff and volunteers
  • Using secure systems for communication
  • Storing information safely
  • Training volunteers on privacy expectations

However, no system is completely free from risk. We encourage clients to share only what feels comfortable and necessary.


 7. Your Rights

As a CHiP client, you have the right to:

  • Request corrections to your information
  • Ask how your information is used
  • Request that certain information not be shared
  • Withdraw consent for information sharing (except where required by law)

We will honor your choices whenever possible.
