Community agencies, churches, counselors, and other service providers may use this form to refer an individual or family to CHiP for assistance.

Submitting a referral does not guarantee services. CHiP will review the referral and follow up as appropriate. This form is not for emergencies. If someone is in immediate danger, call 911.
